A successful recovery often depends on the support a person receives after returning home. Through its discharge initiative, Hale County Hospital extends its commitment to patient wellbeing beyond the walls of the hospital.
Following discharge, hospital fellows will call each Hale County Hospital patient to check in, learn how they are doing, and identify any needs that may affect their recovery.
Conversations about patients’ access to medications and follow-up appointments, transportation and other basic necessities, living situations, and social support help fellows understand each patient’s circumstances and identify ways to support their continued healing.
These calls provide an opportunity to make sure patients know where they can go for additional support, reminding them that their community is invested in their well-being.
Every patient discharged from Hale County Hospital will also be introduced to Project Horseshoe Farm’s Health Partner Program, which promotes social wellbeing through consistent, caring relationships. Through these relationships, patients can receive ongoing social support and companionship as they navigate life after a hospital stay.
Looking ahead, Hale County Hospital hopes to further strengthen the transition from hospital to home by initiating a discharge visit and meal delivery program. These visits would provide another opportunity to check on patients after they return home, offer practical support, and help connect them with resources that promote continued healing.
